The contents of this form will be used to place your order with the vendor and you may request a fax confirmation of the final order prior to the order being placed. Please allow up to two (2) business days for us to process your order and send you a confirmation.

SALES AND RETURN POLICIES:

  • A 50% deposit is required - check, MasterCard, Visa, Discover (must be in the designer's name).
  • Full payment is required upon ordering if goods are to be drop-shipped or picked up by an installer, etc.
  • Balance due upon receipt of goods. Merchandise must be picked up within 10 days from notification of arrival.
  • Designer is responsible for final inspection prior to install and/or fabrication - absolutely no claims on labor.
  • Overage subject to 25-30% restock plus return freight.
  • No cancellation or returns on fabrics, borders, cut rolls, blinds, trims, carpet or furniture.
